D-E Health Office Information

TO REPORT AN ABSENCE, LATENESS OR EARLY PICK-UP
 
D-E uses the SchoolPass app to report absences, lateness, and early dismissals.

To install the app, follow these instructions using your MyDE login email. After installation, you can easily submit attendance and pick-up requests by following the steps here.

While it is appreciated that the health office is notified of your child’s absence or lateness, it is essential that you use the SchoolPass app or contact your division’s attendance office, as they are the record keepers. The nurse does not keep attendance records.

To maintain privacy and confidentiality, email is the preferred method of communication, as phone conversations are limited (and sometimes not possible) when other students are present in the office.

The health and safety of our D-E Families and Faculty/Staff are our top priority. Below, you'll find links to our School’s health guidelines and protocols, detailed instructions for completing health forms through our Magnus Health Portal, our response to COVID-19, and useful health tips to prevent the spread of illness.

ILLNESS & ABSENCES

Please keep your child at home if they experience any of the following symptoms: fever, vomiting, diarrhea, rash with sores or fever, eye infections, or any contagious illness. If your child presents to school with these symptoms, they will be sent home.


RETURNING TO SCHOOL AFTER ILLNESS

Your child may return to school when they have been:
  • Fever: Fever-free for 24 hours without the assistance of antipyretic medication (i.e. Tylenol, Advil, Motrin)
  • Vomiting/Diarrhea: Vomit/diarrhea-free for 24 hours
  • Rash: Free from rash, itching, or cleared by your doctor
  • Contagious Illness: Cleared by your doctor and have been on antibiotic medication for at least 24 hours
  • Eye Infection: Free from drainage or cleared by your doctor
  • Hospitalized: Cleared by your doctor
  • Clearance from your doctor requires written permission to return to school.


RETURNING TO SCHOOL AFTER AN ABSENCE OF THREE OR MORE DAYS

  • A doctor's clearance note is required for absences extending three or more consecutive days (this includes a Friday and Monday absence).
  • The clearance note can be from the primary care provider, telehealth, or urgent care services.
  • The note can simply state that your child is cleared to return to school following illness.


PROTOCOLS AFTER TESTING POSITIVE FOR COVID-19 (AS OF AUGUST 2024)

Attendance
  • You are not required to quarantine after testing positive for COVID-19.
  • Students and staff may attend school and work if they have been fever-free for 24 hours without the use of any fever-reducing medications.
  • Students and staff may attend school and work if they are generally feeling well enough.
  • Please notify the appropriate parties when you are unable to present to school or work (attendance office or supervisor).
  • For students: As per school policy, a doctor's clearance note is required for absences extending three or more consecutive days (this includes a Friday and Monday absence).
  • For faculty and staff: HR may request documentation for absences extending beyond three consecutive days.
Masking
  • Masking is strongly recommended indoors for five days.
  • Masking is required if you are symptomatic.


ACADEMIC & P.E. ACCOMMODATIONS FOR MEDICAL CONDITIONS, PROCEDURES, & INJURIES
 
To better support your child during this challenging period, the student support team is committed to ensuring they receive the necessary assistance and accommodations. For this purpose, we request a medical doctor’s note containing the following information:
  • Outline of necessary medical care, including any medication orders
  • Recommended academic accommodations
  • Description of physical limitations
  • Alternatives in physical education
  • Follow-up or end date. Please note that without a specified date, accommodations may end at any given time.


REST & RECOVERY IN THE MS & US HEALTH OFFICE
 
  • If your child is feeling unwell, they may rest for up to 15 minutes after which they will have to make a decision whether to return to class or go home.
  • If your child requires longer rest for medical reasons, please provide documentation from a medical doctor indicating so (this includes excusal from P.E. classes for discomfort associated with menstrual cycles).
  • Students are welcome to utilize the health office for rest and recovery during their lunch or free periods. However, it's important to note that the health office is not intended for catching up on sleep during class time. Additionally, it is not intended for students to stay in the health office while they are sick if arrangements to go home or stay home are unavailable.


FOOD ALLERGIES & RESCUE MEDICATIONS
 
Food Allergies
Please ensure that your child is educated to ask about the ingredients in foods, especially those that are unlabeled, before consuming them. While D-E is committed to be a nut-free school, the risk of exposure is always present. We ask that everyone in our community—parents, students, faculty, and staff—be mindful of this and avoid bringing nut-containing foods to school. If your child has a severe food allergy and requires epinephrine, please follow these guidelines:
  • Action Plan: An Allergy/Anaphylaxis Action Plan must be completed by your child’s doctor.
  • Medication Storage:
    • If your child cannot self-carry and self-administer epinephrine, please provide a spare epinephrine auto-injector to be stored in the health office.
    • If your child is approved to self-carry and self-administer epinephrine, ensure they carry a spare in their backpack.
  • Medication Expiry: Parents/guardians are responsible for ensuring that the medication remains current throughout the school year or for providing a replacement if it expires mid-year.
  • Emergency Protocol: Regardless of severity, 9-1-1 will be called if epinephrine is administered.
 
Rescue Medications
As with epinephrine, if your child requires rescue medications for asthma, seizures, or diabetes, please follow these guidelines:
  • Action Plan: An Action Plan specific to the condition must be completed by your child’s doctor.
  • Medication Storage:
    • If your child cannot self-carry and self-administer the rescue medication, please provide a spare to be stored in the health office.
    • If your child is approved to self-carry and self-administer the rescue medication, ensure they carry a spare in their backpack.
  • Medication Expiry: Parents/guardians are responsible for ensuring that the medication remains current throughout the school year or for providing a replacement if it expires mid-year.


OVER THE COUNTER (OTC) & OTHER MEDICATIONS
 
Over the Counter (OTC) Medications
The school will routinely stock basic OTC medications listed on the permission form. For information on the requirements and expectations for administering OTC medications to students, please refer to the section titled “Required Health Forms” below.
 
Other Medications
If your child requires medication during the school day for other documented conditions (such as migraines, ADD/ADHD, IBS, etc.), please follow these guidelines:
  • Medications can be stored in your division’s health office.
  • All medications must be accompanied by a doctor's order.
  • Students are prohibited from self-administering controlled medications.
  • Parents/guardians are responsible for ensuring that the medication remains current throughout the school year or for providing a replacement if it expires mid-year.


REQUIRED HEALTH FORMS
 
Every August, there is a wild hurry to submit health forms, immunization records, and reconcile missing signatures to ensure that students can participate in athletics, attend classes, and engage in other activities.
 
*To help facilitate a smooth process for submitting acceptable health forms, please read the entire message.*
 
All students are required to submit updated health forms yearly, as they expire every 365 days. Without valid and current forms, your child may be excluded from athletics, trips, and other activities.

Here are the health forms required every year. Please read below for more information. (*if applicable)
 
  • This specific form is a state requirement for student-athletes. Other doctor-generated forms may supplement but cannot substitute this form.
  • The form has been updated as of June 2024 and now consists of only one page. While older versions of the form are still accepted, all required components must be completed for sports clearance.
  • The form expires 365 days from the "Date of Exam," not from the date it was signed.
Common reasons for form rejection: missing or expired exam date, missing office stamp, or missing signatures. Please note that the form requires TWO signatures by the provider:
  • Cardiac Assessment Professional Development Module: This does not require a physical exam by the provider. The signature indicates that the provider completed this online module sometime in their career. This signature is required for sports clearance per NJ State.
 
Immunizations
  • All students must comply with NJ state immunization requirements by providing proof of vaccinations before the first day of school.
  • New Jersey does not accept the meningococcal vaccine if administered before the age of 10. If your child received this vaccine before that age, please schedule an appointment over the summer to be re-vaccinated.
  • If any vaccinations do not meet the state requirements, such as minimum intervals, you will receive notification that an additional dose is necessary.
  • Immunization compliance:
    • Entering Kindergarten/1st grade: DTaP= 5 doses (4 if 4th on 4th birthday), Polio= 4 doses (3 if 3rd on 4th birthday), MMR= 2 doses, Varicella= 1 dose, Hep B= 3 doses
    • Entering 6th grade: Meningococcal= 1 dose, Tdap= 1 dose. These can be given after their 10th birthday; considered non-compliant if not received by their 11th birthday.
 
  • The State of New Jersey requires signatures from both a healthcare provider and a parent/guardian. The form will be considered incomplete without both signatures and medications will not be administered.
  • All consent must be on the form accompanied by the healthcare provider’s signature, stamp and date.
  • Telephone, e-mail, and verbal orders are not accepted.
  • Common reasons for form rejection: missing signatures of provider or parent, medication has not been checked off.
 
  • Please have the healthcare provider complete this form for any prescription medications that may need to be taken during the school day, including overnight and off school-hour trips.
  • Please send the medication in its original bottle to be stored in the health office.
  • Students are prohibited from self-carrying and self-administering controlled medications.
  • Middle schoolers and the ninth graders have an overnight trip in mid/late September. If your child requires medications during this trip, please complete this form.
 
  • Please have the healthcare provider complete this form if your child has any of the above conditions.
  • A parent’s signature is also required on the forms.
  • A doctor’s order is required for emergency rescue medications. Please indicate if your child is permitted to self-carry their emergency rescue medications and ensure they carry a spare in their backpack.
  • If your child is not permitted to self-carry, please send in the medication on the first day of school. These medications will be kept in the health office for emergency use.
 
Tuberculosis Screening
If your child is transferring from another country, New Jersey may require a negative tuberculosis screening before the start of school; please reach out to determine if this is applicable, depending on the country.


SUBMITTING HEALTH FORMS
 
Magnus Health is the electronic medical records platform D-E uses to manage and securely store student health information.
  • The portal can be accessed under "Resources" through MyDE.
  • Blank health forms are available on the portal.
  • Once logged in, you will see a series of online forms to complete and printable forms for you to bring to your health-care provider.
  • If your child is participating in athletics, another series of sports forms will appear.
  • A yellow star is next to commonly missed signatures, stamps, and other vital information, so please ensure these components are complete before submitting them online.
 
All forms must be uploaded online through Magnus Health. FORMS SENT VIA EMAIL, MAIL, OR FAX ARE NOT ACCEPTED. However, you may fax your health forms directly to Magnus Health using the cover sheet provided on the portal. Please allow a few days for processing.

To access the Magnus Health Portal, D-E families must LogIn to MyD-E. After you LogIn to MyD-E go to the Parent Resource Board for the Magnus Health Portal link (see above visual.)
